By agreeing to the indemnity below:

• I give permission for my child to participate fully in all activities associated with Dapto Anglican Church (Connect Kids), unless I advise the Connect Kids leaders otherwise.
• I give permission for my child to travel in a car driven by an approved Connect Kids leader or a parent approved, unless I advise the Connect Kids leaders otherwise. (Your child will not be in a car driven by a learner or provisional license holder).
• I authorise the Connect Kids leaders, in the event of an emergency, to obtain at my expense any medical, ambulance, rescue or other services that are considered necessary for my child.
• I acknowledge that being part of a community involves mutual care and consideration, and therefore agree that unacceptable behavior may result in my child being sent home and being temporarily or permanently prohibited from attending the activities of Connect Kids.
• I will provide the Connect Kids leaders with any information relevant to the wellbeing of my child prior to him or her attending a Connect Kids activity.
• I give consent for photos and videos to be taken of my child throughout Connect Kids activities and for them to be displayed publicly in print media and on social media.
• I give consent for our family to be contacted by Connect Kids leaders by mail, phone, or pre-arranged home visit, unless I advise the Connect Kids leaders otherwise.
• I confirm that the information given in this form is true and correct, and will advise the Connect Kids leaders of any changes to this information.
